PREDICTING DIFFICULTIES IN LAPAROSCOPIC CHOLECYSTECTOMY USING CLINICAL, LABORATORY AND ULTRASONOLOGICAL CRITERIA: A PROSPECTIVE STUDY

Impact of various clinical, laboratory and Sonological criteria on the outcome of Lapchole, and to identify candidates for open Cholecystectomy. STUDY DESIGN: Descriptive case series. SETTING & DURATION: Era's Lucknow Medical College & Hospital, Lucknow, Uttar Pradesh, India, and Aprajita Hospital, Lucknow, India. From March 2010 until Dec 2014. MATERIAL & METHODS: 792 patients (519 females, 273 males) with Sonological evidence of gallstones in a duration of about 5 years, operated by a single experienced Surgeon team. There are total 15 scores, from history, clinical, Laboratory & Sonological findings. Scores up to five predicted easy, 6-10, difficult, and >10 very difficult. RESULTS: The statistically significant factors for predicting difficult Lapchole were BMI>27.5 (p<0.010), previous hospitalization for acute Cholecystitis (p<0.001), palpable GB (p<0.01) & Thickened GB wall (USG) (p<.037). Predictions came true in 90.2% for easy and 86.2% for difficult cases. CONCLUSION: This scoring system is reliable with a sensitivity of 76% and specificity of 89.2%.
